Request a Referral

In order for our office to process your referral efficiently, we will need the following information:

  • Patients name and date of birth
  • Reason for appointment (child's diagnosis)
  • Insurance information (make sure this is up to date)
  • Name of specialist your child is going to see.
  • Specialist's phone number and fax if available
  • Appointment time and date if it has already been arranged

Once you have the above information, you may request a referral online or if you prefer, you may call our office at 808-263-8822 during office hours and our staff will assist you with the referral process.
