Expecting parents

Congratulations! We welcome prenatal visits to our practice. You may call (808) 263-8822 to schedule a visit. Prior to your prenatal or first newborn visit, please complete our online patient registration. Download and complete the following forms prior to the visit. You may fax (808) 261-6749 or email ilima@reispediatrics.com the completed forms.
Before signing our HIPAA acknowledgement form, please review our section on "Notice of Privacy Practices".

New Patients

Please call our office at (808) 263-8822 prior to registering so we can gather the necessary information and better serve you.

After you have spoken with our office, you may begin the registration process. If you need assistance with the registration process at any point, please call our office for assistance.

Step 1: Register online or download and complete the patient registration. By registering online you will have access to our online appointment and referral requests.

Step 2: Download and complete the following forms prior to your first visit. You may fax (808) 261-6749 or email ilima@reispediatrics.com the completed forms.

Step 3: Obtain a copy of your child's immunizations. Please bring the immunization records to our office, email them to ilima@reispediatrics.com or fax them to (808) 261-6749 prior to your first visit if possible. Otherwise, please arrive 15 minutes early so our office staff will have time to review and input.

Step 4: Appointment time! We look forward to meeting your child. You may request an appointment online or call our office at (808) 263-8822 during office hours and our staff will be happy to assist you.
